With the massive detection suites on board very few have a "good"
explanation.
The vast majority of them ultimately come down to the human element
for one reason or another. (And that's setting aside the simple fact
that if you roll the dice enough times, they're invariably going to
come up snake eyes now and again. Nobody can be 110%, 100% of the
time.) If you're referring to the recent string... Yeah, there's
real problems and the Navy is putting a band-aid over them because
that's all they can do. It's in the hands of the NCA and the Congress
to make real changes and that's simply not going to happen. (tldr;
too few ships, too many commitments, not enough money or time for
training and maintenance.)
All that aside, detection suites aren't magic. Especially in the case
of sonar, they're only as good as the conditions allow.
Re: San Francisco: No, the fish are more nearly amidships. 'Frisco
