I had that very problem once and I had a devil of a time figuring out what was
wrong. It turned out that it had to do with the decolunization settings. I had
turned off decolumnization because scanning tables of contents work better with
it off. Because, at the time, I was scanning one page at a time and there were
no columns on those pages I just left it that way and forgot about it. It did
not even occur to me that might have something to do with a paragraph break
appearing after each and every line. I scanned whole books with that problem
and really put my proofreaders to work. I tried everything I could think of
until I finally tried turning decolumnization back on. Poof! The problem was
fixed.
