It could be simple. Today's adapters come in two pieces: the long cord, and the 
smaller cord with the large box on one end and the plug for the bn on the 
other. Sometimes, the connection between these two pieces gets loose, as the 
long piece works its way out of the converter box. Try checking that this 
connection is tight, and also try a different wall outlet.
