Hey Roger,
You don't need to put in any blank lines. The converter was updated
years ago so that it doesn't accidentally strip out page numbers or
text. You can put in blank lines if you want--but it's extra unnecessary
work, and optional.
The only time that the converter gets confused nowadays is if you have
the exact same identical text in the first line or last line on
successive pages. Then it will keep the line of text on the first page
that has it, but mistakenly strip that text from the following pages,
treating the text as if it were a repetitive running header or footer on
pages that is echoing a chapter heading.

format should be like this. Page break, blank line, page number, blank line, text of page, blank line, page break. It doesn't matter if some of those blank lines are doubled, tripled or have even more blank lines in a row. The converter strips them out anyway. The only reason they should be included is so that page numbers or some text won't accidentally get stripped out too. It is okay to move any page numbers that appear at the bottom of the page to the top of the page, but it is not necessary because the converter will move them for you. If, however, you leave them at the bottom of the page then be sure to leave a blank line between the text and the page number and then another blank line between the page number and the next page break.
